Offered is an authentic 1958 Lake Crystal Farmers’ Mutual Fire Insurance Co. insurance assessment notice from Lake Crystal, Minnesota. This is a wonderful piece of mid-century Minnesota agricultural, insurance, and local-business history.

This was an actively used insurance document, not a blank form. It contains original handwritten policy and financial information, period date stamping, a prominent PAST DUE marking, and an especially interesting series of punched/perforated dot characters across the document.

Interesting Perforated / Punched Coding

One of the most unusual features of this piece is the extensive tiny punched-hole/dot lettering and numbering throughout the form. The individual perforations form recognizable coded characters and appear to represent period insurance/accounting processing information.

These markings are particularly interesting because the notice also contains handwritten policy and assessment information. The punched coding appears to be part of the document’s original processing rather than ordinary decoration or printing.
